You will need to have Adobe Acrobat (PDF reader) installed to open these forms. Just print out the form and fill out as indicated and post to us (address on the form), or give to a committee member.

Once registered and logged in (you need to specify LISA as your choice for donations), you can browse the special offers and visit online shops (such as Amazon etc) and when you purchase something a donation is made to LISA. - For a donation to LISA you can have your Company Logo displayed on this website for 12 months. Email us, or contact a committee member and your Company can be a more visible contributor to LISA.
